The facility was previously operated as Autumn Wind of Louisburg but was closed as result of operational matters by the State of North Carolina in September 2016. We have the approval to reopen the facility from the State of North Carolina with the existing Certificate of Need for 60 beds.

Burnsville is an existing operating 29-bed facility with a history of very high occupancy. We are acquiring the facility in April 2018 from the owner who wishes to retire. The facility has two additional lots that will allow us to increase  housing for independent seniors, and for the future expansion.

Oxford, North Carolina is a 60 bed Assisted Living Facility. Our management company has taken over the management of this facility as of November 2017. Current ownership of this facility has no prior Assisted Living Facility operating experience and is not able to operate the facility.

We are scheduled to acquire this 83-bed facility in early 2018. The facility is currently under a moratorium from the State of North Carolina from accepting new residents. The state placed a moratorium on acceptance of new residents for maintenance related matters.
